Busing Request Form
BUSING REQUEST FORMS WILL BE DUE BY FRIDAY, JULY 7, 2023 FOR THE 2023-2024 SCHOOL YEAR!
Bus request forms NEED filled out for the following:
(PLEASE NOTE: ONLY ONE STOP FOR THE AM AND ONE STOP FOR THE PM, MONDAY-FRIDAY, NO ALTERNATING BUSING FOR DAY TO DAY)
- Recently moved (PLEASE NOTIFY THE SCHOOL YOUR CHILD ATTENDS AS WELL AS THE TRANSPORTATION DEPARTMENT OF ADDRESS CHANGE)
- Child will be getting on/off the bus at a babysitter/daycare location
- Child will be getting on/off the bus at another location
-Child is either returning to in school setting from Cyber or entering Cyber from in person schooling
Complete and submit the form as directed or email completed form to [email protected]
BUSING REQUESTS MAY TAKE UP TO 2 BUSINESS DAY MINIMUM TO GO INTO EFFECT! It will be parent/guardian's responsibility to transport student until new busing starts.
Bus request forms will NOT need to be filled out for:
- Student moving into 7th grade (UNLESS they previously went to a babysitter and no longer need that listed as a stop)
Custody Bus Assignment Request Form
If you have a custody arrangement please fill out the form below and submit it to the Transportation Department every year before school starts so that we can set up transportation for both stops as these do not automatically change over from year to year. Also to make scheduling easier, please include a detailed monthly calendar or note specifying where student needs to be transported to and from on certain days as set forth in custody agreement. PLEASE NOTE THAT IF THE DISTRICT HAS A CUSTODY ORDER ALREADY ON FILE YOU WILL NOT NEED TO SUPPLY THAT AGAIN, WE WILL ONLY NEED THE DETAILED CALENDAR AND FORM SUBMITTED.
