^{Our initial plan in data gathering and planning involves the administration of a survey to parents, teachers, and students grades 3-12 to capture feedback on the essence of the lived school experience.}

 If you wish to exempt your child, please complete the form and submit to building principal.

 

Parents have the right to opt their children out of the survey, and no student will be required to complete a survey.  The entire student survey, parent and teacher surveys, and an opt out form are VIEWABLE below:
