Bus Request Forms – 2026-2027 School Year
Bus request forms must be submitted by Friday, July 10, 2026.
When a Bus Request Form Is Required
A form must be completed if any of the following apply:
- The student has recently moved
(You must also notify your child’s school and the Transportation Department of any address change.) - The student will be picked up or dropped off at a babysitter or daycare location
- The student will use an alternate pickup or drop-off location
- The student is transitioning between in-person schooling and Cyber School (entering or returning)
Please Note:
- Only one AM stop and one PM stop will be approved
- Requests must be consistent Monday through Friday
- Alternating day-to-day busing is not permitted

Processing Time
Transportation changes may take a minimum of 2 business days to take effect.
Parents/guardians are responsible for providing transportation until new arrangements begin.
When a Form Is NOT Required
- Students entering 7th grade, unless there is a change to a previously approved stop (e.g., no longer requiring a babysitter/daycare stop)
SPLIT BUSING DUE TO CUSTODY- REQUIRED PAPERWORK BELOW
If you have a custody arrangement, please complete the form below and submit it to the Transportation Department before the start of each school year. This allows us to arrange transportation for both addresses, as these arrangements do not automatically carry over from year to year.
To help with scheduling, please also provide a detailed monthly calendar that outlines where the student should be picked up and dropped off each day, based on the custody agreement.
Please Note: If the district already has a custody order on file, you do not need to resubmit it. We will only need the updated calendar and the completed form each year.
